Before we can interpret X-ray findings, you need to know what you're looking at. Think of your knee as a complex hinge joint where three main bones come together to create movement and bear weight.

Your knee X-ray shows three primary bones that work together:

The femur (thigh bone) is the large bone at the top of your knee. It's the longest bone in your body and forms the upper part of your knee joint. On X-rays, you'll see the curved bottom end of the femur that creates part of your joint surface.

The tibia (shin bone) is the larger of your two lower leg bones. It bears most of your body weight and connects directly with the femur to form your main knee joint. The flat top surface of the tibia is clearly visible on X-rays.

The patella (kneecap) sits in front of your knee joint like a protective shield. It's actually a floating bone that moves up and down in a groove on your femur as you bend and straighten your knee.

You might also see the fibula, the smaller bone on the outside of your lower leg, but it doesn't directly connect to your knee joint.

Joint Compartments That Matter

Your knee has three distinct compartments, and understanding these helps explain where problems might develop:

The medial compartment is on the inside of your knee, where the inner parts of your femur and tibia meet. This area commonly develops arthritis, especially in people who are bow-legged.

The lateral compartment is on the outside of your knee. Problems here are less common but can occur after meniscus injuries or in knock-kneed individuals.

The patellofemoral compartment is where your kneecap meets your thigh bone. This area can develop arthritis separately from the other compartments.

First, I trace the outline of each bone, looking for the normal curves and shapes. Your femur should have smooth, curved edges at the bottom. Your tibia should have a flat, even surface on top. Any irregularities in these contours can indicate problems. I also check the overall alignment of your leg. Your bones should line up properly – imagine drawing a straight line from your hip through your knee to your ankle. Significant deviation from this line can cause uneven wear on your joint.

Step 2: Bone Surfaces and Fractures

Next, I examine the outer edges of your bones, called the cortex. This should appear as crisp, white lines on your X-ray. Breaks in these lines indicate fractures, which show up as dark lines cutting through the white bone. I pay special attention to areas where ligaments attach, as small fractures here can indicate significant injuries even when the main bones look normal.

Step 3: Joint Space Assessment

This step is crucial for understanding arthritis. The dark space between your bones represents where cartilage should be. Cartilage doesn't show up on X-rays, so we infer its condition by measuring these spaces. Normal joint spaces are several millimeters wide and uniform across the joint. When cartilage wears away, these spaces narrow. This is what doctors mean when they talk about "joint space narrowing."

Step 4: Patella Position

I check whether your kneecap sits centered in its groove on the femur. If it's tilted or shifted to one side, this can cause pain and tracking problems. The patella should move smoothly up and down as you bend your knee.

Finally, I look for signs of swelling or fluid in your knee. While X-rays don't show soft tissues well, they can reveal indirect signs of problems. For example, fluid in your knee can push fat pads apart, creating visible changes on your X-ray.

What "Bone on Bone" Really Means

This phrase terrifies many patients, but let me explain what it actually means and what it doesn't.

Normal vs. Narrowed Joint Spaces

In a healthy knee, your joint spaces are wide and even, indicating good cartilage coverage. The cartilage acts like smooth cushions between your bones, allowing painless movement. As cartilage wears away, the space between bones gradually narrows. In early arthritis, you might have mild narrowing in one compartment. In advanced arthritis, the space becomes very narrow or disappears entirely.

Stages of Arthritis on X-rays

"Bone on bone" refers to the most severe stage of arthritis, where cartilage has worn away completely in some areas. Your bones are literally touching each other without the cushioning effect of cartilage. However, I've seen many patients told they're "bone on bone" when they still have significant joint space remaining. The reality is more nuanced than this simple phrase suggests. Early arthritis might show slight joint space narrowing and small bone spurs. Moderate arthritis typically shows more obvious narrowing and larger spurs. Severe arthritis shows marked narrowing or complete loss of joint space, along with bone changes.

Common X-ray Views and What They Show

Your knee X-ray typically includes multiple views, each showing different aspects of your joint.

Front View (AP)

The anteroposterior or front view shows your knee as if you're looking at it from the front. This view best displays the medial and lateral compartments and can reveal arthritis patterns, fractures, and overall alignment.

Side View (Lateral)

The lateral or side view shows your knee from the side. This view is excellent for evaluating the patellofemoral compartment, detecting fractures that might not show on the front view, and identifying fluid in your knee joint.

Sunrise View

Sometimes called a skyline view, this specialized image shows your kneecap from above, as if the sun were rising behind your knee. This view specifically evaluates patellar tracking problems and arthritis in the patellofemoral compartment.

Signs of Arthritis

Arthritis shows up in several ways on X-rays. Joint space narrowing is the most obvious sign – the dark spaces between your bones become thinner as cartilage wears away. Bone spurs, also called osteophytes, appear as extra bone growth around joint edges. Your body creates these in response to cartilage loss, trying to stabilize the joint. They look like small, irregular bumps on the bone edges. Subchondral sclerosis is another arthritis sign – the bone just under where cartilage used to be becomes denser and whiter on X-rays. This happens because bones work harder when they lose their cartilage cushioning.

Fractures and Injuries

Fractures appear as dark lines cutting through white bone. Some fractures are obvious, showing clear breaks with displacement. Others are subtle, appearing as thin lines that might be easy to miss. Stress fractures can be particularly tricky to see on initial X-rays. These often develop gradually from repeated stress and might not show up until they've been present for several weeks.

Joint Swelling

While X-rays don't directly show soft tissue swelling, they can reveal indirect signs. Fluid in your knee joint can displace fat pads, creating visible changes in the soft tissue shadows around your knee. A special type of joint swelling called lipohemarthrosis occurs when blood and fat from bone marrow collect in your joint after a fracture. This creates a distinctive layered appearance on X-rays taken with a horizontal beam.

When to Seek Medical Care

Understanding your X-ray findings helps you know when to seek prompt medical attention. You should contact your doctor immediately if your X-rays show: Any type of fracture, even if it seems minor. Some fractures that look small can become displaced and cause serious problems if not properly treated. Signs of infection in the bone or joint. These might appear as areas of bone destruction or unusual bone formation patterns. New or worsening arthritis symptoms that don't match your X-ray findings. Sometimes the X-rays lag behind your symptoms, and additional imaging might be needed. You should also seek care if you have severe pain that doesn't match what your X-rays show. While X-rays are valuable, they don't tell the whole story about your knee health.

Lifestyle Considerations After X-ray Results

Your X-ray results should inform your activity choices and treatment decisions, but they shouldn't necessarily limit your life. If your X-rays show early arthritis, focus on maintaining knee strength and flexibility through appropriate exercises.
